The monument is located on U.S. Route 40, across from the Nemacolin Country Club, Beallsville, Pennsylvania (about twelve miles east of Washington). Access to the monument is available from a small roadside pull-off.

This monument was dedicated on December 8, 1928. The monument was rededicated in October, 1978.

Inscriptions are found on all four faces. The inscriptions read:

A bronze plaque has been placed
at the foot of the monument. It reads:
